Step 3: Practical Sign-Off

The critical final step. The carer performs the task independently while being formally assessed by a qualified clinical lead. The assessor must sign off that the carer is safe, confident, and competent to perform the task without direct supervision.

Client-Specific Competencies

In domiciliary and complex care, competencies should often be client-specific. A carer deemed competent to manage PEG feeding for Client A may need a separate sign-off for Client B, whose equipment or specific needs differ.

Evidencing the Process
